  • DLA/New forms arrived!

    My son's DLA ends on 14 Jan 2013, and today I received the forms.

    So I have contacted the department of Disability Alliance (DAP) who helped me the last time to ask if they still have copy of the old forms, as nothing's changed either way.

    Just waiting for them to get back to me now by email.
    I did also have a copy but cannot for the life of me find them.

    Looking through the forms, as they are still the standard dla forms and not yet PIP, but I have to admit I do not find them so daunting as the children's dla forms. As these ones I have to complete are for adults. Golly my youngest will be 18 in January 2013, scary!

      Had a nose on the National Austism Website, and there is also a Supplement form to enclose with the dla form, (optional) but if you are an appointee for your child moving into adulthood (mine is 18 in Jan 2013), with autism it should all be taken into account on when they are making the decision.

        Forms half completed, heard from my son's specialist the other day, she is sending through a support letter as well by next week.

        These forms I do not find as difficult as the child form to be honest, still a pain but not as difficult.
        I also have a supplement form questionnaire in regards of autism to enclose with the dla form, that I have already completed.

        Not looking forward to the new PIP though, bet they will be tough.

          Good luck with the application, hun. Are you going to scan and save the forms?
          I'm still keeping my head down and hoping they forget about me for a few years till I'm past 64...but somehow I doubt it!

              Yes I am going to save them, what I've done is downloaded one pdf directly from the site, and the one they sent as a spare one, looks neater using the processor lol, then I shall save the online completed form, and print of the one as it has to be signed manually, they can have that one.

              Fingers crossed for you hun too.
              They will be narrowing down over the next few years until the PIP fully takes affect. xx

                  Yes PIP will be tough as it is not designed to help the disabled but purely to cut the funding and remove the reference Disability from DLA.

                  You will be pleased to know that the date for implementation of PIP has been put back a few times and it is no where near ready to be implemented before the end of this parliament so your sons new award may well stay put a while.

                  BTW there is similar issues with Universal Credit too.

                    I have lost touch with the PIP issue at present, will have to get back into it though to keep an eye on it.

                    This current award he is on was awarded from the age of 12 to the day before his 18th birthday, was very surprised he had that time to be honest, but the new one if they decide to award him I assume it would be for an indefinate period, because of the new PiP coming into action in due course.

                    Well, the online form is completed, am debating whether to copy that into the hardcopy or just keep that one as spare, as there is no date actually on the form when to say when to send back, but on the letter that came along with it, it does say to post back as soon as possible.

                    So when is the best time?
                    Runs out 14 Jan 2013.

                      I woulnt assume that. My friend has an indefinite award, and has been told to expect a review for PIP. Not to say it would happen, but it may - even though was indefinite

                          ****** has been attending my Community Paediatric clinic, and at the request of his parents, I am writing to support their application for Higher Rate DLA.
                          He has also been under the care of Torfaen CAMHS, and has a diagnosis of Asperger syndrome, which is an Autistic Spectrum disorder.

                          ****** continues to have great difficulty with social interaction. He cannot interpret social situations, and feels acutely uncomfortable if he has to meet new people. He cannot cope in a group, not knowing what is expected of him, and because he interprets everything in a literal way is unable to be sure he has understood. The level of anxiety this creates in him is so intense that he panics and has to have the reassuring presence of a familiar adult to help him. Most of the time he will avoid this type of situation altogether, which means he remains very isolated and requires virtually constant supervision.

                          Although his school made a lot of effort to adapt his class size and provide support for him , he eventually had to leave because he was so anxious that he would go missing from lessons. He has been educated at home with a private tutor.
                          He is unable to deal with financial affairs, such as benefits, etc., and needs help with letters, filling in forms etc.

                          When out, he has to be accompanied by one of his parents. He cannot go out alone. He cannot cope with loud or busy environments.

                          He still requires help with personal care, not because he is unable to do the tasks, but because of sensory sensitivity issues, and poor personal organisation.

                          His parents have to be constantly re-assuring him, and helping him to calm down. At times he becomes very distressed, and has emotional outbursts which need very sensitive handling.
                          This is clearly very emotionally draining for his parents.

                          I definitely support his parents' application for Higher rate DLA, because of the level of support ****** requires from them. Please contact me if you require any further information.

                            DLA forms with extra information sent.........

                            Dla form, Autism supplementary questionnaire, support letter/info from his Specialist.

                            Recorded delivery (1st class), they should receive it by tomorrow.
