Strange search by APEX

While going through my credit reports, I found this very random search by a company called APEX. I dont recall applying for anything around that time and all my defaults I have held since 2005 so surely it wouldnt be related to any of them as all 3 are satisifed.

Says I have 2 defaults in the last 36 months but this isnt true because the actual dates on the same report show this cant be the case. Also the Hutchinson 3G one isnt listed as a default.

Ok So a brief rundown... Lloyds tsb me and DH both have 2 UE cards with them.. :UE
We also have a loan and an OD with them. (those were with CAB repayment plans)
however the account in which the OD was with has been closed (by them) and the Loan and OD have gone to different... -
