If a debt has gone to court and the CCJ states a set amount you have to pay every month, can you still make an offer of full and final settlement? An offer that would be less than the full amount owed.

Yep course you can.
If it's with 28 days then the ccj also gets wiped!
Make any offer to the claimant.I'm the forum administrator and I look after the theme & features, our volunteers & users and also look after any complaints or Data Protection queries that pass through the forum or main website. This would actually be quite appealling to the creditor I would imagine as they get a lump sum immediately rather than having to wait years, and a debt of say £1000 will be worth far less in five years time.
Also there is a real boom industry at the moment in selling old CCJ's for enforcement as the OC have trouble collecting (despite it being through the court).
