02 too many credit checks! Equifax

Back in April I applied to 02 for a contract phone, a Blackberry phone, on checking my files with Equifax there are 3 full credit searches for the one phone. Surely 2 must be duplicates?
Should I tell Equifax first or get in touch with 02 first? I got the phone but bit naughty 3 searches and full searches.
