    Okay dont worry, just get it off to the CEO, and on looking into this further, you then realised that a pre-med condition applies to you :-) X

                  Just give them further details of meds taken if this applies, and anyone you have seen under your condition, maybe a specialist etc, you do not have to literally give addresses or anything or names of specialists.

                  So if the med condition was for eg a year before the policy was taken out then write the year approx.

                  If there was nothing discussed with you about med conditions at the time the policy was sold to you, then you just need to say "the medical side of things were not discusssed, because if it had then I would have confirmed then about my pre med conditions"

                  In regards of the 3rd question, you need to write that you understand that having a pre med condition could void a claim on that particular health condition where this condition is still ongoing, and I have also taken leave from my job due to this condition.

                    So I need to write back (or can I email?) and basically say:
                    I had a back problem that started c1997, that flares up sporadically to this day. I was given pain killers called 'A', and anti-inflammatory medication 'B', and I was a physio at the hospital, on one occasion I had to call out a doctor in the night on who gave a shot of 'B' by injection to ease the extreme pain. at a later date I was referred to a private physio at 'B' practice, who said it was a muscle problem.
                    Over the years I've had to take time off work due to this condition.
                    The medical side of things were not discussed when I was sold PPI, because if it had then I would have confirmed then about my pre med condition.
                    I understand that having a pre med condition could void a claim on that particular health condition where this condition is still ongoing, and I have also taken leave from my job due to this condition.
