    Yes that is correct and why I said if you get the claim right from the outset with all the info there should be no need for an assessment.

    I see you have liked this group too so look in the Notes and Files as it is breaming with good help and advice too

          Not yet Di but is due so may be worth giving DWP a call to find out when so the new form does not overlap your claim

              I have not had chance to check the online form but will be doing this later, but did ring for an appointment for my son at my GP to get a note, when I spoke to the receptionist she said to check on the form if there was anything the Dr must complete as well? Can anyone confirm this please?
              I said its just for a note but she said to make sure before giving an appointment.

                On the ESA 50 there is which goes into more detail and will follow once he is in the assessment phase.

                      Right, I cant get a Dr's appt for my son until 3rd Feb unless they get a cancellation before that!! I take it they will only pay out once they have the note and will not accept the claim form without that? Other than that i do have evidence from hospital specialists he was under?

                        I would carry on as your are for now and gather evidence and get the form filled in as that will take you a couple of weeks anyway Di, there is nothing stopping you sending the forms off with a note saying sick note/evidence to follow but be warned DWP is in chaos right now and have heard of documents going missing so do not send originals only copies.

                        Include contact details of any and all medical professionals your son has had contact with

                          Thank you Pompey.
                          The form will be completed by Friday with help from local welfare company,and all other info almost ready.

                          So am I best off waiting for the sick note before sending the claim form? Questions,Questions sorry lol, x x

                            Do you need a doctor's appointment to get a sick note Di, surely they know about his condition/problems? I'd phone the surgery again and explain what you want.

                                You can send everything you have off with a note stating due to backlog at GP's cannot get appointment until 3rd Feb so that will follow and be sent soon as have it, it is bloody ridiculous though that someone with a lifelong condition needs a sicknote as all the evidence you will be sending is more than enough.
